Fixed Penalty Notice (FPN) Disclosure

I am applying for Tier 2 (General) Visa from outside the UK
I have one Fixed Penalty Notice (FPN) for Littering two years back, which I have duely paid off immediately at that time. I don't have the FPN copy with me (since it was 2 years ago and I must have misplaced it), but I have the FPN Number and the details of my payment along with the transaction ID and amount.
Three questions:
1. Shall I disclose this FPN in my Visa Application.
2. What evidence must I provide for it. Will the FPN number and Transaction details suffice?
3. Will this result in refusal of my Visa Application.
