Supporting documents received but no letter

I applied for my tier 2 Visa September 28th. I mailed my documents into the New York office through VFS. I received an email 2 days ago saying a decision had been made.

today, I received my passport and all my supporting documents back but no visa and no explanation letter.

Does this mean I have been denied?

can you see a vignette for entry clearance in the passport?
