PBS Tier 2 dependant extension 2nd time

Hi,
Very urgent!!!! Appreciate your support and help.

I have committed drink drive offense on 09/01/2018. I have completed driving disqualification on 09/10/2018. I have completed 5 years on tier-2 dependant route. Do i need to go for tier 2 dependant extension again? Or can I apply for ilr. If i go for tier-2 dependant extension what is the visa end date i need to mention in the application because my spouse got ILR on 14-12-2017.
