Medical Treatment and Ni questions for Tier 2 Applicant and Dependent

Dear folk,

I, main applicant, have been received CoS for Tier 2 sponsors in the UK and we are now in process of making applications for both main applicant and wife as PSB dependent but we ran into some issue below and would like to seek for advise.

1. Should we make the application in the same time? We have no issue with maintenance fund.

2. I used to have Tier 2 study visa, Tier 1 Post-Study work and Tier 2 General but left UK in 2014 (visa expired 2015) and go back UK twice on 2015 and 2018 as visitor visa. I did go to hospital in 2013 which was for tooth extraction which was free but I did not indicate that I had medical treatment for last two visitor visa application as I did see it as serious medical treatment. My question is how should I fill in Tier 2 application form? on Medical treatment? - this apply for my partner too.

3. We wife used to have Tier 4 study then changed to Tier 1 Post Study Work and left the country in 2013 and she used to have part-time job but she has not have record for her NI number. She have been back to the UK for many times as visitor (apply for visitor visa) but on NI question, she always put as she does not have NI number and visa is granted every time so now she is applying for PSB dependent. Should she said that she has but indicate that she does not have record for NI number. (forget)

Normally, case worker checks on application current entry clearance or check the history of information that we gave to them?
