I've submitted my app for ILR and 2 dependents as PBS dependents
my questions are

is there any "consent form" for dependent application ?

which docs to upload in "proof of application" section?

current passport and consent form. If you read closely it mentions the same.

I am not aware of the PBS dependent form. If there was, the form would have given you a link to download. There is a consent form for SET(O) ILR I answered for that above.
