Information/requirements for a dependant visa application(Tier 4 students)

I am a Tier 4 student and my wife is also. We both are africans from different nationalities and we are government sponsors. And also, we are not citizens of the UK.
We got married on 24th August, 2019 outside the UK and are now back to complete our courses of study.
I am currently on a course that will end this november, 2019 and my visa will run out on 22nd January, 2020. Although my wife has a longer stay since her discipline is medicine, which is why I need to apply for a dependant visa to stay back with her after my course ends. Her current tier 4 general student visa ends November 2020.
My enquiry is concerning how to go about applying for my dependant visa, the documents needed as well as the fees required.
She also hasn't changed her surname yet to my surname as it will take about 10 weeks to change her names on the passport. So I wonder if this will affect the visa application ?
I am appealing to you moderators or anyone who has gone through this process lately to kindly assist me in providing the necesaary information in order to have a hitch free application please!
