urgent qurey: hsmp dependent application on 25-08-2008

Hi,
I got my HSMP in Jan 2008 from Pakistan. I have been working in UK for past 4 months. I am now making my wife's visa application on 25-08-2008. I have couple of questions?

1. What VAF form to use?
2. I have written a covering letter listing everything i am providing and a sponsorship letter from lawyer as well. Does my wife need to write a covering letter as well, as she has her tb certificate, bank statements as well which is not listed in my covering letter.
If yes, will she be mentioning again about docs provided by me??
