A letter of consent from Parents for Children under 18

Hi there, For online naturalisation application, where can I get consent form for my Child?
At the end of the application, only referee declaration forms are available to download.
Please share with me if you have any formats.

Here you go... courtesy @Cullinan . Similar format is available on MN1 guide I believe but it didn't have address field.
