UKM Application rejected due to lack of barcode?

Hello all. I'm an American applying for UKM (British Mother) citizenship, but I'm running into issues starting my biometrics appointment here in the states. My ASC office will not accept my current appointment form because it has (what they call) an outdated barcode that they say cannot be scanned by their machines. Has anyone heard of this problem? Any help would be greatly appreciated.
