Questions on taking a second job and filing self assessment

Hello,

I appreciate that this question has been asked multiple times and I have checked the replies from many of them, but there does not seem to be a definitive answer.

I am currently working for the NHS in a profession that is on the shortage list. I checked with HMRC before I took the second job, which is the exact same profession but in a private sector, and HMRC told me that it is not an issue as long as it's less than 20 hours per week.

My second job is not PAYE. I worked for a private service where I am allocated to see patients privately, the organisation takes 30% of the fee and I am seeing as an associate by the runner of the organisation.

So my situation is summarise as below:
1. My profession is on the shortage list
2. My second job is not PAYE
3. I cover less than 20 hours in my second job

Have I still breached the tier 2 condition given the above factors? Thank you!
