Previous applications on the citizenship form

Hi everyone,

I'm trying to complete the citizenship application form in gov uk. I'm now at the point where it asks me about my previous applications. I have two questions:

1- Do I list all of my immigration history with UK here? I was on the Tier 2 Skilled Worker route, so do I list them all, starting from my first job, other job changes and eventually the Indefinite Leave to Remain application?
2- The form asks "Name in which you made the application", I'm assuming this is my name at that point in time? I got confused because there is nothing else on the form to indicate the type of immigration type I was pursuing, like changing jobs, getting Tier 2 visa or ILR application.

Yes - if you had to apply for a new visa for a job change include that as well, otherwise just the one visa application, not familiar with work visas.

Yes it is your name, otherwise it would have asked which application it was.
