Request for Return of Documents

Can anybody help me regarding the process of the Request for Return of Documents for an application made for the transfer of CoE-RoA.

It has been more than 10 days since I requested for the return of my documents.

I would like to enquire to find out what is happening but unable to locate the "Progress enquiry form".

Has anybody used it before?
