Tier 2 Extension for Dependent Child Inputs

Hello ,
Tier 2 extension for dependent child. My family has been in the UK for 4 years 8 months now . As there are short of 5 years , i am applying Tier2 extension visa for them .

I have clarification on the following
1. 'Have you travelled to UK in the last 10 years?'
Since my family is currently in the UK and applying for the Extension , do we need to say Yes or No ..
I am not sure if we need to specify the entry clearance information . I have already entered in the travel history(others) for the visits we did to our home country .

2. Medical Treatment history in UK
I am unable to recollect all the appointments we had done for the past 4.8 years. Could you add the recent one and mention in the cover letter that all the treatments had been covered under the IHS .
