Limited Leave to Remain Granted due to medically unfit to fly

My father was granted limited leave for 30 months and the extracts of the HO letter are as below:

Consideration has been given outside the Immigration Rules and due to your particular
circumstances that you are medically unfit to fly due to your medical conditions it has
been decided to grant you a period of leave outside the rules on an exceptional basis.

As his 30 months limited leave to remain is coming to end, my question is: which form to use for extension? Please share link for online application.
