FLR Tier 1(HSMP) form help

Dear All,

I am planning to apply for my HSMP extension (1+3+3) along with my wife's dependent visa in a single form (FLR Tier 1 - HSMP) as per JR. My confusion is that will the HO consider both the extenstions made in one form as one application or two applications.

More specifically, What should I fill in for the question 5 in "Details of applicant(s)". Should I say 1 or 2? My understanding is that I should mention it as 1 and specify the no. of passports as 2 after my surname in the question 7 and no need of listing my wife's name in question 7.

Is that correct?

Also do we need to give the CVV no. if we are paying by mastercard?

You need seperate application form for Teir 1 dependent....
