In terms of past visa applications, some of my history goes back to 2003 when I did a masters but then left UK. For that, I have no idea what the application reference number was or what date I had applied. Can I just roughly mention the year and skip day/ month of application?
Try to give a best guess of the month. It doesn't count towards your statutory requirements regardless.
For the referees, do NHS radiographers count as professionals? They are not specifically on the list.
If not on the list, no.
