I am not sure how to fill in section D5 to D10.

I am on student visa at the moment. I've lost my passport 1 yr after my study and I got issued a new passport with a new student visa since then. Can some1 help with the following questions:

1) D5 - as my lost passport with the old student visa is not available for the applicaiton I guess I will have to write Yes and move to D6 isn't it?

2) D6 to D10 - is that referring to my latest entry to UK or is it about my first flight to UK with my old student visa? I don't think I keep the details about my first flight to UK which is 3 years ago.
