waiting 4 months to get FLR and yet no reply

Dear All,
my wife has applied to increase her visa(FLR) on tier 2 with her certificate of sponsorship number and other documents. 4 months have passed and yet she hasnt got her passport back. automated message on the inquiry line now says that one have to wait atleast 14 weeks before they can inquire. she did it and they replied, it wasnt yet complete. Without her passport she cant travel either.

Does anyone have a similar experience? Why will a simple FLR take more than 4 months all after paying a handsome application fee?
