Applying from India - query in filling appendix form

In pbsappendix form section 4.8 ,
1. For Source of earning , its asking for details like Employer name, Business, start and End dates of the source of earning(refer the details in bracket), my question is how to include all the details in this small box
2. In Earnings is pounds column should I put the exact of conversion of my Indian earning or I should put the Salary equivalent (after using multiplication factor)
As in points based calculator (refer below) its Earnings in pounds shows only direct coversion of my Indian salary.
for example
IF
Indian Gross salary is 900000
Exchange rate 0.013
Earning is pounds is 900000 x 0.013 =11700
Salary Equivalent after multification factor is 67860
Now which data i should give in the form section 4.8

Try to write as small as possible. If some letters come out of box, it should not be a problem.
