HSMP extension -Urgent Help Please

Hi,
I am looking forward to apply my HSMP extension this saturday.
Please someone help me in following point!!!!

Main Applicant Form - Page ::: 20

D5. Are either the applicant’s current and/or any one of his/her previous passport(s) or travel
document(s) that he/she has used to travel to, and remain in, the United Kingdom which
shows his/her current leave unavailable?


I have enter first time in UK 27-April-2008 (approx. 17 months ago) then what option i suppose to select YES/NO ???

Did you lose or change your passport in the last two years? If not - then the answer is NO.
