Initial application Tier 1- Doctor needing help

Please could friends clarify/advise.
My wife is currently a doctor in training post but on the old system work permit issued to her in 2006 when the permit-free visas were stopped.
Her current workpermit visa issued 4 yrs ago has no restrictions on training as it was meant to allow her to complete training.
My query is regarding an initial application for Tier 1. My understanding is that she should not get a stamp restricting her from continuing her training(last few months left) as her current entry clearance as WP holder issued 4 yrs ago to take up this post had no restrictions. Am I mistaken or missing something?
I request experts to kindly advise.

Perhaps not, as WP restricts employment to a specified employer. See also 245D(d)(iii)(2).
