Dependent (Spouse) Name Change

Dependent (Spouse) Name Change

Hi all

My wife's name changed after marriage. Her name in the passport now includes my name as surname while in the marriage certificate she only has her first name. Should I provide the affifavit that name changed due to marriage or the simply the marriage certificate is enough?

One more question, is it good to attach the degrees of my spouse as that's not the requirement?
