Tier 1 dependent - Application questions

Request inputs on following questions about my daughter's application
1) No place of issue mentioned on her passport. However we got her passport by a post office in city near Chicago, USA.
2) Main applicant's normal country of residence - If I have been in US from last 4 years on work VISA. Should I mention USA as residence country or country of my citizenship?
3) Main applicant's employment details - I am just applying for tier 1 VISA and do not have any employment information (such as company name, job begin and end date etc) in UK. Should I mention that I am not employed for the purpose of this question?

Will appreciate help in resolving above queries. Thanks
