Fixed Deposit Certificate- Complicated

Hi All,
Recenty I applied for my psw, I was refused and I didnot get my original FD certificate.
Can I apply freshly with certified true copy?
Also the FD certificate start date is somewhere in Nov. I have renewed it in Nov. I got a letter from my financial institute stating that I had this money for 90 consecutive days- renewal dates.
Will I be refused again?
Please be kind enough to shed some light on this issue.
