Tier 2 to Tier 1 switching application - help needed

Hi,

I am applying for a switch from tier 2 ICT to tier 1 general. The application form has two fields:

1. migrant reference number
2. worker reference number

I'm not sure what these mean - can someone please point me to whether they apply to me, and if yes, how I can find them?
