student visa

Is the student visa and Entry Clearance two seperate applications? I have seen posts regardig applying for student visa, then waiting for Entry Clearance. Or maybe I have been reading so many posts I have become confused. Also, is there any way to apply for student visa MORE than three months before course study begins?
Thanks in advance fo clearing this up for me.
