changing name, should I keep old name as middle name?

changing name, should I keep old name as middle name?

My child will apply for a British passport. I plan to change my child's name to an English name and keep her previous name as middle name. However it seemed that there is no middle name on passport application form. Is it useful to keep a middle name? Or should I put her first name and middle name together as "given name" on passport application form?
