which application form??flr(m)?flr(o)or set(m)

Hi every 1 again
Ok I searched but couldn't get proper answer; the answer I was looking was which application I should use for my wife’s visa extension!
Here is our history

I came to UK as an International student in july2000 and my wife joined me here in UK in dec2001. I applied for long residency last year which was given in Oct2010 but before that I had student visa which going to expire on 30/04/2011, so because of that I didn’t apply for my wife visa extension, because there were about 9months or so left in student dependant visa. Now I am going to apply for her extension and we have already in hand LIUK certificate (which is I think not needed at this point) also have English certificates like level 1 and level 2.
MY question is that which application form should be used I asked many people like (citizen advice bureau, a different solicitor who also deals with immigration matters) they said (flr(o) and set(m) ) but in my thinking I should use FLR(M).
Please can anyone put me into right direction please?
