Tier 1 - Dependant Visa questions - application form

Hi friends,
i am new to this forum and referred by one of my friend..gone through several posts..many of my questions were answered, but still have few more and posting here.

brief about me: i am here in uk from last 4 months on Tier-1 Visa (expires on Mar 2012).
my family lives in india .wife + kid .now i wanted to make application for my wife as dependant , not for my kid.(will bring my kid after my visa extension in mar 2012, due to some my personal reasons). my wife changed her passport after marriage .so she has 2 passports in hand.
i am trying to help my wife in filling application and has few doubts. we are using VAF 10 form. here are my questions..

1) 1.3 Other names (including any other names you are known by and/or any other names that you have been known by) - do we need to fill old passport name here (which mention her old surname) or leave it as Blank?
2) 2.5 Is this your first passport? should i keep here as NO..and give old passport details or should i say YES here ???
3) 7.19 Main Applicant: When is the job or course of study expected to finish? --i am on permanant role here. what date should i keep here..my visa end date or leave it blank??
4) in the application, they didn't ask about the children and we are not mentioning any where in the application or in cover letter.... would it be problem when we apply for my kid visa later..?

Please clarify these .

Mention her maiden name i.e. name prior to marriage

Keep 'No' and give previous passport details

If possbile say Not application or leave it blank

It should not be a problem, as long your are not lying and using deception. If you were not asked about your kids then you do not need to reveal it.
