PEO Tier 1 Extension - Evidences and clarifications

Hello Everyone,

My PEO appointment for tier 1 extension is confirmed for 30th of June.
Currently i am on HSMP and planing to provide following evidences for the points. please advice me if you think i am missing some info/evidence.

1. Age : Passport will be provided for evidence

2. Qualification : Original bachelor Degree + transcripts + UK NARIC Letter (same as of initial HSMP application)

3. Earning : 12 Salary slips (printouts signed and stamped by Employer) + original Bank statements of last 12 months + Employment letter from my company. (i have P60 as well but its not covering full 12 claimed months)

4. English Language : currently on HSMP and has previously satisfied the English Language requirement. (i have a copy of letter from university saying that my degree was thought in English not original letter)

5. UK Experience : same employment letter and salary slips.

6. Maintenance Fund: same bank account statements as provided for earning.

i have following questions.

1. what should i mention in, Section 7, Summary Sheet "documents provided(please list)" in application form for English language requirements? as i am currently on HSMP and has previously satisfied the English Language requirement and i am afraid i lost original letter from university that i submitted with my initial application, though i have copy of it . shall i get a new one?

2. salary slips print out signed and stamped by company HR are valid or shall i request on company letter head only?

3. shall i add p60 as well though its not covering full 12 months period.

4. shall i request my HR to provide me my salary history for claiming 12 months in tabular form showing gross and net salary or its not required?

any further advice or suggestions in this regard will be highly appreciated
