Family Permit - Declaration

Hi,
next week we are going to Rome to ask Family Permit
and they asked me also a "written, signed and dated declaration of the EEA national confirming that (s)he will accompany the non-EEA national"

I'm looking for a sample letter declaration,because I don't know
what to write.....
Somebody could help me?

You simply state that the non-EEA national will accompany you. Just as adviced.

Shouldn't matter as long as it is legible. Signature must be by hand of course (don't copy an image of it to your electronic document!).

I would write something like:
I am an [Italian] citizen. I will be traveling with my spouse [NAME] to the UK.

my wife got EEA permit!

When we arrive in London immigration will ask something to us?
common questions like :
what are we doing in UK?
how long do you stay?

no,I didn't give any photos.
The most important was the declaration and bank statement
