Tier-2 ICT Dependent Child Visa online application

Hi Everyone,

I am applying Tier-2 ICT Visa for my baby in chennai, India. I have got Tier-2 ICT Migrant visa on 09/04/2010 and its valid until 11/04/2012. I am unable to decide the input while filling up online Tier-2 ICT dependent visa for my new born baby. Could you please tell what should I choose on this below online application fields.
