mother's uk visit visa documents checklist, any help plz

Hi
I am on tier one, will be eligible for ILR in Feb, 2013. I am living here with my wife and daughter. I want to apply for my mother's visit visa so that she could see her grand daughter for the first time.

What documents will I need.

Applicant (My mother-in-Law)
VAF1B Application form.
National ID Card

( Mum is a widow, spent her whole life as housewife. dont have any property in her name)

Passport Copy ( do I need to send my HSMP approval letter as well.)
Pakistan National ID Card Copy
Employment Letter
Wages Slip
Bank statements (It's joint account with wife, can it be a problem?
or should we write a joint sponsor letter, what?
Tenancy Agreement ( Recenctly moved so dont have any bills in my name yet)
Daughter's Birth Certificate from UK

Do I need to show any other document from Pakistan to prove relationship. Like Form B from Nadra.
and how many months bank statements and wages slips are required, 3 or 6.
And finally should my mother go for TB test in Pak.

I shall be thankful for any suggestions.
