Contracting 'Opt Out' form

Hi guys,

Recently, I have got a contracting offer. They have also included 'Opt Out' form along with Contract. I will contract through my own ltd company.

Could you please help me to know if I should sign the Opt out form? On some forums, I have read I should prepare my own Opt Out form with the help of accountmant and sign, they say I should not sign Opt out form prepared by agency.

Please help.

It's not really for me to say 'yes' or 'no' as at the end of the day it is your choice - so long as you know the pros and cons of both sides. If you haven't read it already this site is a good place to start:
