Tier 1 extension queries

I am filling the Tier 1 general Main and Dependent application forums and came up with the following queries, would be great if somebody could help me on these.

Tier 1 general Main Application

1. Please enter the reference number on the entry clearance or visa which can be found in the applicant’s passport or travel document?


2. I am also filling my wife application as my dependent, On Main applicant form there is a declaration for joint account. Do i need to get her signature as well although the utility bill is on both of our names but i am sending it only as part of her dependent application and has nothing to do with the main applicant form.

Dependent application form:

1. On F10,F11,F12 of the dependent form , they asking for figure prints take , although her figure prints were taken in Pakistan but don't know what should i write on the related questions below:

F10. Has the dependent had fingerprints taken as part of a previous United Kingdom immigration application made in the United Kingdom or abroad?#
F11. Give details when fingerprints were taken:
F12. Give details where fingerprints were taken, including the town or city and country.
F13. Give details of the British diplomatic post(s) involved if the application(s) was/were

This number is available on the top right corner of the visa stamp. It is the number in red

R u sending the dependant form separately? The dependant has to sign on the dependant application form anyway.
