Reference letter from Account format -Tier1 Ext

Hi All,

I am going for the tier1 extension next month. I Planning to collect the
1. Dividend Voucher
2. Salary Slips
3. Referene Letter from the Accountant.

I am not sure if my accountant has? Please can some one provide me the format of it..
