Doesn't have a surname in passport

Hi,

My wife doesn't have any surname in her passport or any certificate. I have to apply for Tier 1 dependent visa for her. Whether it will create any issue?

If not, what should I mention under surname as it is mandatory while filling it online.

And again why does she not have a surname/family name? I thought it was a mandatory part of getting the passport to begin with?
An [Expensive] Immigrant journey has ended 19/08/2015. Good luck to you all out there...
