Regarding First enter in UK and old passport for SET(O)

I got my Working Holiday Visa on 07/06/2005 and came to U.K. on 07/07/2005. Then switch over to HSMP on 21/05/2007 which was valid up to 07/06/2009 Then after extended visa to Tier 1 General on 21/05/2009, which is valid up to 07/06/2012.
I am preparing for my ILR Application, Please advise on following queries :

1. Regarding Section 6.1 of SET(O) which is :
When did you (the main applicant) first enter the UK? This refers to the date of your first entry into the UK at the beginning of the period of stay on which this application is based.
Please guide what should I fill here????


2. My old passport had the initial Working Holiday Maker visa stamp, But after expiry, got it renewed from London, they cancelled the old passport and make a note on new passport that the old passport is cancelled and returned back to me because it has a valid visa, Then after when I change my visa to HSMP, obviously they put visa stamp on my new passport, I couldn’t find my old passport now, Is it mandatory to produce the old passport with SET(O) or only the current will do? Please Guide….

1)the first entry into uk date..
2) it is mandatory to produce ALL Passports that you have used to travell to UK, if any is absent u must produce a police report or somthing substantial offical document explaning the reason for loss/damage/theft of the relevant passport.

Now if I put the very first entry date, then in next secion i.e. 6.2 they ask about any absence from U.K. since that date, It means I am going to wright down the absences from U.K in last seven years which total will be more than allowed, regardless they will have to calculate absences for only last five years, I don't think so that will be a problem????????

In the form you should detail all entry/exit dates since u FIRST entered uk......................then write a cover letter explaning that they should only count last 5yrs , show your calculation of gaps/absences...provide reasons if u can .......any other details/confusion parts provide explanation in your letter........ that should clear things to the case woker......
