PBS Dependant extn - Travel Doc Number Urgent Help!!

For my partner's extension of LTR, I'm filling
PBS Dependant Application Form (Version 02/2012) and stuck on:
page (13)
C3. Do you have any passport or travel document which contains a previous grant of leave or which you have used when you last travelled to the UK?

If YES, complete table below... which asks Passport/Travel Document Number.

I am wondering if I need to supply here just the Passport number or current visa reference number as well.

I need urgent help on this. Please respond.
