Mix of Visa Categories - ILR Qualification

Hello
I checked the existing topics but couldn’t see relevant response hence posting it.
Here are my details :
1. Tier 2 (General Category) visa issued to me on 09/11/07. I entered in UK on 05/12/07
2. Renewed Tier 2 visa by same employer but under category (ICT) T2SW visa, which was issued to me on 07/09/09
3. Got Tier 1 (general category) on 22/11/2010 which is valid till 22/11/2012
I have few points for clarifications:
1. Do I need to count my 5 years from first visa stamping date or date of entry
2. Tier 2 ICT categories was issued before 6th April 2010 but at that time there was no provision of taking period of leave as there was no role/work changed for me. Does this definition given in 245CD (combination of visa categories under Tier 1) would have any implication on my case?
3. Tier 1 issued to me is a “residence permit” but so far I and my family never got a request for biometric. Do I need to get the same prior to apply for ILR? I expected the same while applying for Tier 1 also but then I got the visa stamped on my passport directly and UKBA never sent me any request for the same.
4. Can I qualify for ILR? If yes, which time frame I should consider for taking an online appointment. Pls see above dates prior to suggest
