I guess if both of you are under Tier 1, either of the two can write the Sponsorship letter
2.can i write the covering letter on behalf of my baby because baby is too small cant write.and how should i start that covering letter for example I THE MOTHER OF(my baby name) WANT TO APPLY FOR TIER! DEPENDENT VISA ON BEHALF OF MY BABY.. OR should i write directly from baby side???
The child cannot write the letter, hence the sponsor (parent) can write the letter on the child's behalf.

Well if you have a T1D visa then you can't write the sponsorship letter, your husband has to do that. You can still write the cover letter for your child as you would be applying on your child's behalf.
The document list looks okay. Utility / phone bills are not mandatory but could still be included as additional supporting documents and please don't include vaccination booklet of baby.
