dilema - appeal urgent

i have my appeal index pack and witness statement that need to be sent off by tomorrow before 12 and i have been re reading a letter and i think i need to make another copy, but my question is.. who do i send them to??

the tribunuals service in stoke?
first tier tribunual in loughbourough?
or my court place in nottingham?
or to the UKBA?

im so confused i know i only need to send off two packs now but to whooo? please help :(
