Which application form is needed to fill in to extend the visa EEA family permit, staying in the UK?

The Family Permit does not have an expiry date as such but a 'use by' date.

Once the 6 month expires it does not affect your status if already in the UK but if a person leaves after it has expired, then it can no longer be used to re-enter the UK. A new application has to be made and used within 6 months of issue.

In the UK an application for a residence card can be made. Outside the UK an EEA family permit.
