New born child visa for UK Work permit

Hi,
I have valid UK work permit to work in UK and currently employed. Recently I am blessed with a baby in UK. I have obtained Indian Passport for baby. I would like to know the process to obtain UK visa for my baby who is staying with me. I have searched for the form on http://www.ukba.homeoffice.gov.uk/ but could not get it. if any one can provide correct link to the form and details of fees that would be helpful. I am planning India trip so need to urgently apply for UK visa for my baby.
