Tier 1 extension application for contractors

I am about to apply for an extension of my Tier 1 Visa.
I have been working as a Contractor since past one year and have a Limited company registered with me as the Director. I take a small salary as well as dividend(4 times the salary) from the company so as to reach a figure of 42K for my earnings.

Could anyone please guide me on what documents I need to show as a proof of my earnings?

Are the company formation documents needed as well?

Also do I fall under self-employed category? I am confused as I draw a salary every month and dividend as well.

A format of Letter from Accountant would be of great help :)

Congratulations to you. I will definitely follow other threads, but it will be a great help if you can summaries the list of documents you have provided to claim points for your earnings.
