From 6 April 2012 we will be removing the provision for applicants in the UK to apply for permission to stay here (known as leave to remain) before they have taken or received the results of an English language test. This policy applies to applicants applying under Tier 1 (General), Tier 1 (Entrepreneur) and Tier 2 of the points-based system. In tier 1 ext you HAv to pass the test again?

No because you have already satisfied the criteria in your initial Tier 1 application
Check Section 4 of the form and point 131 of the guidance notes which states
An applicant meets the English
language requirement, without the need to
provide evidence, if he/she was last given
permission to stay in the United Kingdom:
•under Tier 1 (General) and is applying for
an extension of leave to remain;
