visa for my new born baby?

Deal All,
I am on UK work permit for 5 years (2007 Jan-2012 Jan).my wife is on dependent visa. we have baby in january 2010. So I need to apply for visa & passport. I have enough info for passport which i can apply at indian embassy in london.

ANYBODY PLEASE ADVICE ME WHAT IS THE PROCEDURE TO APPLY FOR MY BABY VISA.

1. which application form we need to fill for my baby( my work permit 2007-2012 is old system )

2.what documents required

please share any useful information or weblink or contact numbers.

need this info bit urgent
