Would it be required to list au pair arrangements and internship on an unpaid base, is that considered employment?

First priority is for you to complete all employment details, to the best of your knowledge.
Whether your employers have reported your employment correctly or not at all is not under your control - they would have to account for their actions. Clearly UKVI have access to HMRC records and may raise queries at their discretion.
