Non-European Mother of of EU citizen daughter

Hello All

My 8 years old daughter is an EU citizen and I am not. Just wondering if somebody would be able to help on which form should I fill in order to ask for a right of residence for me please?

Also, which EEA member-state is she a citizen of? Are there any other family members involved?
