Help related to wife's visa application

Hi,
I am a British citizen. My wife recently submitted a FLRM visa extension application. She submitted and signed the part 1. For part 2 and 3 she wrote not applicable. I (husband) signed the declaration for spouses form.
I am currently not employed and she works as a dentist and has her income covered at the level mentioned by the home office.
Today she received an email stating: We are unable to conclude your application and we need
1. Consent form pages 2/3 (I am assuming this is part 2 and 3 of consent) to be signed by the sponsor
2. Copy of the full completed SA302 tax return from 2019-20 and 2020-21 to verify earnings (she had submitted these letters from our tax advisor when she made the application).
Do I (spouse/husband) need to fill/sign or she has to fill/sign part 2 and 3? We have her tax letters which we can easily email.
Any help would be much appreciated.
